The blood–brain barrier (BBB) prevents the brain uptake of most pharmaceuticals. This property arises from the epithelial-like tight junctions within the brain capillary endothelium. The BBB is anatomically and functionally distinct from the blood–cerebrospinal ...

Interessante citaten:
Drug distribution into CSF is frequently used as an indicator of BBB transport. This is a misconception that stems from the lumping of the BBB and BCSFB systems as a single brain barrier. In fact, the BCSFB is leaky compared with the BBB, and
all molecules in blood enter the CSF at a rate inversely related to molecular weight (MW).
Certain small molecule drugs may cross the BBB via lipid-mediated free diffusion, providing the drug has a molecular weight <400 Da and forms <8 hydrogen bonds.
In summary, small molecule drugs may be reengineered to enable transport across the BBB via the CMT systems expressed in the brain capillary endothelium. Large molecule drugs, including recombinant proteins, and peptide and antisense radiopharmaceuticals may be delivered across the BBB with MTHs that target the endogenous RMT systems expressed within the brain capillary endothelium. The reengineering of pharmaceuticals for BBB transport is enabled with knowledge on the endogenous CMT and RMT systems within the BBB.
BCSFB: blood–cerebrospinal fluid barrier
CMT: carrier-mediated transport
RMT: receptor-mediated transport
MTH: molecular Trojan horse
The central nervous system (CNS) is tightly sealed from the changeable milieu of blood by the blood-brain barrier (BBB) and the blood-cerebrospinal fluid (CSF) barrier (BCSFB). While the BBB is considered to be localized at the level of the endothelial cells within CNS microvessels, the BCSFB is...
