The authors used a rabbit model of subarachnoid hemorrhage to demonstrate that a single dose of alpha lipoic acid could reduce lipid peroxidation (MDA levels reduced), improve superoxide dismutase and glutathione levels, reduce the death of cells to apoptosis (programmed cell death) and reduce the severity of cerebral vasospasm. Erdi MF, Guney O, Kiyici A, Esen H. The effects of alpha lipoic acid on cerebral vasospasm following experimental subarachnoid hemorrhage in the rabbit. Turk Neurosurg. 2011;21(4):527-33.
Nitric oxide (NO) depletion and inflammation contribute to the pathogenesis of cerebral vasospasm. L-Citrulline increases L-arginine levels, raises nitric oxide synthesis and reduces cerebral vasospasm in mouse models of subarachnoid hemorrhage. L-Citrulline is also safe to use. Pradilla G, Garzon-Muvdi T, Ruzevick JJ, Bender M, Edwards L, Momin EN, Thompson RC, Tamargo RJ. Systemic L-citrulline prevents cerebral vasospasm in haptoglobin 2-2 transgenic mice after subarachnoid hemorrhage. Neurosurgery. 2012 Mar;70(3):747-56; discussion 756-7.
Curcumin in multiple doses is effective against glutamate neurotoxicity and oxidative stress and improves the mortality rate in rats with subarachnoid hemorrhage. Vasospasm remains an extremely serious complication that affects patients presenting with subarachnoid hemorrhage (SAH) due to ruptured intracranial aneurysms. In rat models of subarachnoid hemorrhage, the administration of N-acetylcysteine significantly reduced vasospasm and can represent a new therapeutic alternative. Pereira Filho Nde A, Pereira Filho Ade A, Soares FP, Coutinho LM. Effect of N-acetylcysteine on vasospasm in subarachnoid hemorrhage. Arq Neuropsiquiatr. 2010 Dec;68(6):918-22.
Magnesium lithospermate B (MLB), an extract from Salvia miltiorrhiza, was effective against coronary artery disease, ischemic stroke, and chronic renal disease. This study examined the effect of Magnesium lithospermate B on endothelin-1/endothelial nitric oxide synthase (eNOS) in a subarachnoid hemorrhage animal model. The magnesium lithospermate B group of rodents did not show vasoconstriction in the basilar arteries. The enhanced expression of eNOS and decreased endothelin-1 levels in the treatment groups may reflect its anti-spastic effect. This study shows that magnesium lithospermate B modulates endothelin-1 levels through a NOS-dependent mechanism in the pathogenesis of cerebral vasospasm following subarchnoid hemorrhage. Chang CZ, Wu SC, Kwan AL, Hwang SL, Howng SL. Magnesium lithospermate B alleviates the production of endothelin-1 through an NO-dependent mechanism and reduces experimental vasospasm in rats. Acta Neurochir (Wien). 2011 Nov;153(11):2211-7.
The combination treatment of insulin and vitamin C effectively relieved the cerebral vasospasm in a rabbit model of subarchnoid hemorrhage, possibly through increasing the insulin receptor α expression and nitric oxide levels, whereas treatment with either insulin or vitamin C alone failed to do so. Li S, Xue J, Shi J, Yin H, Zhang Z. Combinatorial administration of insulin and vitamin C alleviates the cerebral vasospasm after experimental subarachnoid hemorrhage in rabbit. BMC Neurosci. 2011 Aug 1;12:77.
Alpha lipoic acid reduces oxidative stress and maintains BBB function in rats that experienced subarachnoid hemorrhage.Erşahin M, Toklu HZ, Cetinel S, Yüksel M, Erzik C, Berkman MZ, Yeğen BC, Sener G. Alpha lipoic acid alleviates oxidative stress and preserves blood brain permeability in rats with subarachnoid hemorrhage. Neurochem Res. 2010 Mar;35(3):418-28
Melatonin was investigated as a neuroprotective in a rat model for subarachnoid hemorrhage. The BBB showed increased oxidative stress in the rats. When melatonin was administered, the effects were reversed. Brain water content was diminished, BBB function was improved and brain edema was reduced. Ersahin M, Toklu HZ, Cetinel S, Yüksel M, Yeğen BC, Sener G. Melatonin reduces experimental subarachnoid hemorrhage-induced oxidative brain damage and neurological symptoms. Erşahin M et al. J Pineal Res 2009; 46(3): 324-32.
Hyperbaric oxygen therapy has been shown to ameliorate brain injury in a variety of animal models including focal cerebral ischemia, global cerebral ischemia, neonatal hypoxia-ischemia and subarachnoid hemorrhage. Hyperbaric oxygen therapy is associated with improved cerebral oxygenation, reduced blood-brain barrier breakdown, decreased inflammation, reduced cerebral edema, decreased intracranial pressure, reduced oxidative burden, reduced metabolic derangement, decreased apoptotic cell death and increased neural regeneration. On a molecular level, hyperbaric oxygen therapy leads to activation of ion channels, inhibition of hypoxia inducible factor-1alpha, up-regulation of Bcl-2, inhibition of MMP-9, decreased cyclooxygenase-2 activity, decreased myeloperoxidase activity, up-regulation of superoxide dismutase and inhibition of Nogo-A (an endogenous growth-inhibitory factor). As research expands on hyperbaric oxygen as a therapy, its clinical applications should follow. Matchett GA, Martin RD, Zhang JH. Hyperbaric oxygen therapy and cerebral ischemia: neuroprotective mechanisms. Neurol Res. 2009 Mar;31(2):114-21.

Resveratrol may prevent damage from subarachnoid hemorrhage.
Subarachnoid hemorrhage, a bleeding in the space between the brain and the membrane around the brain, is a serious and life-threatening condition with critical time limitation in treatment. People who do survive a subarachnoid hemorrhage will often suffer from significant cognitive and physical impairments.
One of the more detrimental effects of a subarachnoid hemorrhage is cerebral vasospasm, in which constriction of blood vessels cause tissue death in the surrounding brain areas. Past research has found that resveratrol, a natural substance found in red grape skin and other plants, can reduce vasospasm—making resveratrol a popular supplement for people looking to improve their cardiovascular health.
A recent animal study found that resveratrol administered soon after a subarachnoid hemorrhage reduced both cerebral vasospasm and tissue death. Reseveratrol may therefore be one of the few interventions that is both safe and effective for subarachnoid hemorrhage, potentially reducing both the risk of death and improving outcome after survival. traditional Chinese medicine such as Salvia miltiorrhiza, Acanthopanax senticosus, Ginkgo biloba, Pueraria lobata, Liguisticum chuanxiong, cow bezoar, Diospyros kaki and Gynostemma pentaphyllum have been proven beneficial in CVS prevention and treatment, while Salvia miltiorrhiza and TCM soup have unique effects on bleeding absorption. In addition, aescine and some TCM soup might relieve strong headache after SAH.
